[ Summary ] About 20 years have passed since the first report of CT colonography. In the last 20 years, CT apparatus has made significant progress in acceleration and multiplicity of the detectors, leading to high-resolution CT images unimaginable before. With recent advances in CT colonography techniques and bowel preparation methods including fecal tagging, accuracy of CT colonography has been validated by many large-scale clinical trials. The number of deaths from colorectal cancer (CRC) is still increasing whereas the checkup rate of colonoscopy following positive fecal immunochemical tests is not increasing in Japan. Therefore, CT colonography is expected as an alternative to colonoscopy. In 2012, the Japanese government (the Ministry of Health, Labor and Welfare) approved medical insurance covering CT colonography for a CRC screening tests, suggesting increasing numbers of CT colonography for CRC screening. In Japan, however, standardization of CT colonography procedures and educationa1 programs for the readers are both lacking in Japan. Therefore, we should start addressing these issues without delay.
