[ Summary ] Helicobacter pylori (H. pylori)‒negative signet‒ring cell carcinomas present as pale intramucosal lesions. We have previously reported that based on the MIB‒1 labelling index that serves as a marker of proliferative potential, H. pylori‒negative signet‒ring cell carcinoma shows lesser proliferative potential than H. pylori‒positive lesions. We described 90 patients who underwent endoscopic submucosal dissection (ESD) in such cases, and only 2 patients showed submucosal (SM) invasion. In contrast, ESD performed in patients with undifferentiated lesions in H. pylori‒positive cases revealed SM invasion in % of patients. Based on our data, lesions measuring > 15 mm in size grow whole mucosa.
