[ Summary ] The use of laparoscopic surgery has now spread widely in Japan and the number of the patients who receive laparoscopic surgery is expected to increase in the future. A few phase‒III trials may support the use of laparoscopic distal gastrectomy (LDG) as a standard treatment option for clinical stage I‒III distal gastric cancer. A phase‒II trial to evaluate the safety of laparoscopic total gastrectomy (LTG) for clinical stageⅠ gastric cancer confirmed the safety and feasibility of LTG in terms of the incidence of esophagojejunal anastomotic leakage. However, no studies have proven whether LTG is the oncologically equivalent to open surgery. The short and long‒term outcomes of LTG for advanced gastric cancer patients still remain unclear. Although several limited surgeries for early‒stage gastric cancer patients using Endoscopic submucosal dissection, Endoscopic full thickness resection, and Laparoscopic and endoscopic cooperative surgery (ESD/EFTR/LECS) techniques have been reported, we need the accumulation of the further date to obtain evidence of its clinical benefits.
