[ Summary ] The use of endoscopic submucosal dissection (ESD) for early gastric cancer has been spreading with advances in devices and techniques. From the results of the multicenter prospective study (JCOG0607), expanded indication for differentiated‒type gastric cancer in Gastric Cancer Treatment Guidelines 2014 changed to absolute indication in the Gastric Cancer Treatment Guidelines 2018. The other multicenter prospective study (JCOG1009/1010) will reveal the long‒term outcomes of ESD for undifferentiated‒type gastric cancer. Now, elderly patients are increasing. We should discuss the indications and additional surgery for early gastric cancer in elderly patients considering not only factors regarding lesions but also factors of individual patients.
