[ Summary ] The first edition of Japanese gastric cancer treatment guidelines was published in 2001 as the first cancer guideline in Japan and had an important impact on treatment decisions that were previously made without scientific evidence. Since then, clinical trials have been conducted using the guideline recommendations and these have produced new evidence. The guidelines were first written in a Textbook style and have been transformed to the Evidence Based Medicine style based on rapidly emerging significant evidence, especially in the chemotherapy field. In the latest 5th edition, each section of surgery, endoscopy and chemotherapy includes new recommendations and clinical questions. Chemotherapy regimens for advanced disease are now categorized into "Recommended regimens" and "Conditionally recommended regimens", which provides a wide range of treatment choice for various patient conditions.
