[ Summary ] Acute cholangitis often progresses to a severe form, which may precipitate sepsis. It is essential to confirm the pathomechanism and severity of acute cholangitis. Additionally, it is important to comply with the Tokyo Guidelines 2018 outlining the treatment strategy for acute cholangitis. The etiology of acute cholangitis is classified as benign inflammatory conditions (biliary stones, sclerosing cholangitis, and duodenal diverticulitis) and malignant diseases (biliary cancer). Endoscopic sphincterotomy (EST) is not always required for this condition. The effect of biliary stenting for patients with acute cholangitis does not differ in the choice of between endoscopic nasobiliary drainage (ENBD) and endoscopic biliary stenting (EBS). Recently, endoscopic ultrasound (EUS)-guided biliary drainage or the rendezvous method has been attempted to replace ENBD and EBS.
