[ Summary ] Although various drainage methods are reported for the management of malignant hilar biliary obstruction, there is lack of consensus regarding an optimal stenting strategy for malignant hilar biliary obstruction. Currently, symptomatic patients (those with jaundice among other symptoms) with unresectable malignant hilar biliary obstruction, usually undergo endoscopic transpapillary biliary drainage, such as plastic inside stents or multiple metallic stents. Recent advances in anti-tumor therapy have led to improved prognosis in patients with malignant hilar biliary obstruction. Clinicians should choose the stenting method considering the feasibility of re-intervention in addition to the initial stent patency. Endoscopists should perform a biliary stenting procedure only after confirming the biliary stricture characteristics using imaging studies.
