[ Summary ] Novel pathological classification of pancreatic neuroendocrine neoplasm (PNEN) was recently published in the WHO 2017 Blue Book on Endocrine Tumors. Major changes from WHO 2010 classification in this book are summarized as follows: PNEN is first classified into pancreatic neuroendocrine tumor (PNET) and pancreatic neuroendocrine carcinoma (PNEC) based on morphological features. PNET is then further sub-classified-into PNET G1, G2, and G3 based upon the status of tumor cell proliferation, and PNEC into small and large cell carcinoma according to their morphological features. Recommended method of obtaining Ki67 labeling index was also proposed in WHO 2017. Nomenclature of the tumors harboring both endocrine and exocrine differentiation was proposed as MiNEN (mixed neuroendocrine non-neuroendocrine neoplasm), instead of MANEC (mixed adeno-neuroendocrine carcinoma).
