[ Summary ] Liver transplantation for hepatocellular carcinoma has been recommended for Child-Pugh C considering the patient's condition and Milan criteria considering the tumor condition in the guideline revision, which is an insurance indication for liver transplantation of brain death or living body. About twenty years have passed since Milan criteria report, and new criteria have also been reported. However, as a guideline, it is low in evidence and has not been recommended. There is no new recommendation of evidence for downstaging and treatment for recurrence after liver transplantation. However, regarding the prevention of recurrence, it is suggested that mammalian target of rapamycin inhibitor suppresses recurrence, and in Japan the use of immunosuppressants after liver transplantation may be reconsidered.
