[ Summary ] Endoscopic submucosal dissection (ESD) is one of the treatment options for elderly patients with superficial esophageal cancer. The Charlson Comorbidity Index was used to evaluate the risk before an ESD. Our data showed a low incidence of adverse events and the five‒year‒survival rate in elderly and oldest patients was 87.4 % and 87.5 % respectively. Therefore, ESD has been shown to work well for disease control in elderly patients, although the risk of comorbidity needs thorough evaluation before ESD, and we need to choose a well‒balanced additional treatment option after the ESD.
