[ Summary ] Endoscopic resection for early gastrointestinal cancer has been underwent in the world. In particular, endoscopic submucosal dissection (ESD) enables en‒bloc resection and accurate pathological evaluation for large lesions. Bleeding and perforation are the most common complications in ESD. Incidence of intraoperative perforation is reported to be about 0.5‒5.3 % in gastric ESD, 4.1 % in colon ESD, and 19 % in duodenum ESD. The prevention of complications can be ensured by clear visualization during the procedure, which contributes to the safety and reduction of the duration of the ESD procedure. It is important to be aware regarding the complications of ESD and the characteristics of internal organs before performing ESD. If the complication has occurred, it is essential to perform endoscopic hemostasis or closure with due attention to the general condition of the patient. This prevents further complications from occurring.
