[ Summary ] The indication of the need for hypopharyngeal endoscopic mucosal dissection (ESD) is a superficial squamous cell carcinoma (SCC) without lymph node metastasis (LNM). Preoperative CT and neck US are necessary to detect LNM and an intubated general anesthesia is necessary to perform a safe ESD.
The techniques of mucosal resection and submucosal dissection are same for esophageal ESD. However, hypopharyngeal ESD is difficult to perform owing to the complicated anatomical structure of the hypopharynx.
Nonetheless, a clip-with-line technique is useful for safe and effective dissection. Moreover, because it needs experienced and skilled endoscopists, hypopharyngeal ESD should be performed in a high-volume center.
