[ Summary ] For the diagnosis of Barrett's esophageal adenocarcinoma by using Narrow Band Imaging (NBI) magnifying endoscopy, the diagnostic criteria for gastric cancer are usually applied. International classifications of magnifying endoscopy findings have been reported ; however, the diagnostic accuracy is insufficient. A new Japanese classification is needed. Cancer development under the squamous epithelium can be diagnosed by observing annular vascular formation and the small white sign. Although NBI magnifying endoscopy is useful, this method has some limitations. Negative biopsy around the lesion is helpful to improve the diagnostic accuracy for the tumor margin.
