[ Summary ] In the past, in Japan, radiographic screening using upper gastrointestinal radiography with a barium meal was the only method recommended for gastric cancer screening. In the revised 2015 guideline, endoscopy was newly recommended for population‒based and opportunistic screening of the upper gastrointestinal tract. Meanwhile, it has become clear that Helicobacter pylori infection is the major risk factor for the development of gastric cancer. Although there is a possibility that the Helicobacter pylori antibody and serum pepsinogen methods, representing the 'ABC‒methods', can be used for the risk stratification of gastric cancer, these methods were not recommended for population‒based screening. Therefore, careful consideration is required before introducing these serum markers for gastric cancer screening.
