[ Summary ] Endoscopic papillary balloon dilatation (EPBD) is widely used for the management of bile duct stones (BDS) in Japan, whereas its use has been mostly abandoned in Western countries because of the risk of post‒procedure pancreatitis. EPBD is a particularly useful option for patients with coagulopathies. Several previous studies showed no significant difference in the rates of complete stone clearance and complications in simple cases with a few small stones. In addition, the late complications such as cholangitis and recurrence of BDS are rather less frequent with EPBD than with endoscopic sphincterotomy (EST). When the indications are appropriate and the procedure is performed carefully, EPBD may be more advantageous than EST.
