[ Summary ] As the elderly population has been growing in Japan, the number of elderly patients requiring endoscopic treatments for common bile duct (CBD) stones has gradually increased. Endoscopic treatment is the first choice of treatment. Even in patients with advanced age, CBD stones should be treated. However, very elderly patients often have chronic diseases such as cardiovascular disease or dementia, and thus, it is often difficult to treat them, even endoscopically. As there were no differences in the treatment efficacy and complications between high age group and non‒high age group, we should perform endoscopic treatment for CBD stones in very elderly patients under strict risk management, and under such conditions the method is effective and safe.
