[ Summary ] Acute colorectal obstruction often accompanies colorectal cancer and requires urgent treatment. Despite the existence of self‒expandable metallic stents, preoperative drainage using a transanal tube still enables elective surgery for obstructive distal colorectal cancer. Our report suggests that technical and clinical success rates for procedures involving transanal tubes were 93.9 % and 86.4 %, respectively. Use of transanal tubes resulted in a high rate of clinical success and enabled safe one‒stage surgery for colorectal cancer with acute colorectal obstruction. In this paper, we describe methods, some tips, and preoperative management of the drainage using a transanal tube.
