[ Summary ] A number of clinical observational studies providing evidence on the efficacy of continuous regional arterial infusion (CRAI) therapy for severe acute pancreatitis (SAP), including a randomized controlled trial (RCT), have been reviewed. A recent clinical study (propensity score matching analysis) using the Diagnosis Procedure Combination (DPC) data showed no significant difference in the mortality rate and incidence of intervention between the CRAI and non‒CRAI groups. The revision committee of the Japanese guidelines for acute pancreatitis recommended that the CRAI therapy should be ungraded because its efficacy in the treatment of severe acute pancreatitis has not been confirmed. However, a recent meta‒analysis on the CRAI therapy showed significant differences in the mortality and complications between the CRAI and non‒CRAI groups. Further studies regarding the CRAI therapy are required.
