[ Summary ] ABC classification allows assessment of gastric cancer risk and may be useful for gastric cancer screening when used in an appropriate combination with imaging procedures. Maniwa city in Okayama established a system for gastric cancer screening based on the ABC classification in cooperation with the medical association and administrative agencies, and has successfully detected early gastric cancers. Since early gastric cancers have been detected by endoscopy not only in the year when we conducted the ABC screening but also in a subsequent year, this multi‒year screening system appeared to be working. However, a concern was raised about the low screening rates for the initial test using ABC classification and the second test, which mainly used endoscopy. Misclassifying subjects with a previous or persistent infection with Helicobacter pylori in Group A should be avoided by all efforts. Basic implementation procedures including quality control should be provided in order to successfully introduce and disseminate ABC classification into community screening.
