[ Summary ] For patients with liver cirrhosis due to hepatitis C virus (LC‒C), adaptation to interferon (IFN), and its efficacy in treatment has been limited. However, after 2014, new therapeutic strategies have been provided, such as interferon‒ free regimens with direct‒acting antivirals (DAAs). We could use these regimens for patients with compensated LC‒C, and these are in fact recognized as the first choice of treatment for these patients. However, their efficacy would still not be sufficient in the treatment of LC‒C patients, so strategies such as prolongation of the treatment, combination therapy with ribavirin, and exclusion of patients with resistance‒associated variants (RAVs) could be considered. Additionally, serious adverse events such as decompensation as a result of these treatments need to be avoided. It is therefore evident that more safe and effective treatments are urgently needed for treating LC‒C patients.
