[ Summary ] At first, the only device available for endoscopic submucosal dissection (ESD) was a needle knife, it was impossible to perform esophageal ESD safely. A handmade preliminary hook knife was developed in 1999, and the first ESD was carried out safely. I have presented lectures and published papers about esophageal ESD. However, the procedure was not widely accepted because of its difficulty. Therefore, I conducted ESD live demonstrations in 2003 to teach all ESD procedures, from diagnosis, submucosal injection, and mucosal incision to submucosal dissection. Then, ESD became a popular procedure, and the Japanese insurance system started to cover esophageal ESD in 2008.
A clip‒with‒line method was published in 2002, which is now widely accepted for obtaining good traction during ESD. In addition, a new hook knife with a water jet system was recently established this year. It is a unique knife that makes ESD easier to perform.
