[ Summary ] As well as ESD for esophagus and stomach, recently safety and effectiveness of colorectal ESD has been proved. In April 2014 national health insurance started to cover reinvestment of ESD for malignant colorectal neoplasia. As there are many adenomatous lesions as precursor of carcinoma, we have to select an adequate treatment method such as EMR (including piecemeal resection) or ESD, conduct precise endoscopic diagnosis prior to treatment and decrease complications. In these situation Japan Gastroenterological Endoscopy Society (JGES) created the EMR/ESD guidelines including the indication of EMR/ESD, preoperative diagnosis, treatment technique, judgment of curability after endoscopic resection, complication, prognosis, pathologic diagnosis and so on.
