[ Summary ] In first version of guideline of acute cholangitis and acute cholecystitis 2005 and Tokyo Guideline 2007, acute cholangitis cannot be diagnosed by imaging diagnosis.
However, the recent update, TG13, emphasizes the usefulness of contrast enhanced dynamic computed tomography (CT) for correct imaging diagnosis of acute cholangitis and cholecystitis. In patients with acute cholangitis, dynamic CT shows patchy inhomogeneous enhancement of the liver on the arterial dominant phase. This characteristic inhomogeneous enhancement may disappear after improvement of acute cholangitis. In patients with acute cholecystitis, dynamic CT shows pericholecystic contrast enhancement of the liver.
