[ Summary ] Over the past thirty years management of gastroduodenal diseases has been revolutionized by the discovery of the Helicobacter pylori. In Japan, a dramatic decrease in the number of patients with peptic ulcer diseases (PUD) has been documented, resulting in a marked reduction in the medical spending associated with PUD. Furthermore, insurance approval for treatment of H. pylori gastritis as recommended by the Japanese guidelines for management of H. pylori infection has triggered a sharp increase in the number of patients receiving eradication therapy. This will lead to a dramatic decrease in the number of gastric cancer cases in Japan. The remaining issue is the management of functional dyspepsia. To this end, we need further research to elucidate the brain‒gut‒microbiota axis, which can be applied not only for the management of gut diseases but for other non‒communicable diseases such as metabolic syndrome, cardiovascular diseases, allergic and immunological diseases
