[ Summary ] Endoscopic submucosal dissection (ESD) for colorectal tumors is more difficult than that for gastric tumors. It is most technically difficult to remove lesions with submucosal fibrosis. Fibrosis into the submucosal layer is caused by several factors. It is important to evaluate the state of fibrosis before starting ESD. When the submucosal layer can not be identified, as when there are views of muscle traction, the procedure should not be continued.
This procedure requires a high level of skill for operators due to its technical difficulty and higher risk of perforation. It is important operators understand the technical difficulties presented by these lesions.
