[ Summary ] Endoscopic submucosal dissection (ESD) of the large intestine began to be covered by National Health Insurance in Japan in April 2012. However use of this technique has not sufficiently spread due to technical difficulties involved. Its further acceptance is expected because ESD is considered safer for rectal lesion treatment than for colic lesions. ESD in the large intestine is most advantageous for lesions near the anus in regard to functional conservation. However, full knowledge of points to note in relation to characteristics of the region near the anus and resolution of these difficulties is necessary.
