[ Summary ] Colorectal endoscopic submucosal dissection (ESD) has been approved by the national health insurance system since April 2012, and its use is becoming widespread. However, the procedure is sometimes difficult because of the risk of perforation due to its anatomic features such as narrow lumen, thin colorectal walls, and the presence of semilunar folds. Concerning the application of colorectal ESD, institutional limitations have been removed. However, some technical difficulties have not been resolved. To overcome technical limitations, many ESD blades are now available. It is important for endoscopists who perform colorectal ESD to understand features of such particular ESD blades, and to select adequate blades in relation to endoscopistʼs skill levels and intraoperative situations.
