[ Summary ] The newly developed guidelines termed the "Guidelines for Gastroenterological Endoscopy in Patients Undergoing Antithrombotic Treatment" is a milestone in understanding multiple risk assessments not only for endoscopic procedure-related risks but also of non-endoscopic related risks. Unfortunately, because of a lack of high quality clinical evidence, the evidence level for statemensts contained is not high and recommendations are not strong enough as provided in these guidelines. This means the guideline is an unstable one and can be modified by newly found high quality evidence which will be discovered in this field. Continuous research, periodic re-evaluation, and revision of the guidelines will be necessary.
