[ Summary ] Although the number of deceased donor liver transplantation (DDLT) has been gradually increasing since amendment of liver tansplantion bill, majority of liver transplantation still depends on living donor liver transplantation (LDLT) in Japan. 70 % of LDLTs are adult-to-adult cases. Recently, the number of patients with hepatocellular carcinoma (HCC) has been increasing. While Milan criteria are a gold standard for indication of HCC, several extended criteria have been reported from Japanese institutions and good results were reported. Although right lobe graft is used mainly all over the world, left lobe graft has been reconsidered because of the safety of donors, especially in Japan. Survival rate is almost same between LDLT and DDLT, but poor prognosis was reported in LDLT for HCV cirrhosis from an old donor or that for ABO incompatible case.
