[ Summary ] Diseases involving the hepatopancreatobiliary system including primary sclerosing cholangitis (PSC), autoimmune hepatitis (AIH), cholangiocarcinoma, gallstones, autoimmune pancreatitis (AIP) are encountered in patients with inflammatory bowel disease (IBD). PSC is most commonly associated with IBD complications, especially ulcerative colitis (UC) and complications from PSC, which are considered to be associated with the pathogenesis of UC. However, the frequency of PSC as UC complications in Japan is not as high as that observed in Western countries. Moreover, IBD with PSC complications has distinct characteristics and is somewhat different from the typical clinical features of UC. The prognosis for PSC with UC complications is the same as that for PSC alone. Currently, there is no effective treatment for PSC and liver transplantation may be required.
