[ Summary ] Toxic megacolon associated with ulcerative colitis is a very serious condition, frequently requiring surgery. Medical treatments using corticosteroids, anti-TNFα antibodies, or tacrolimus produce improved results. On the other hand, patients frequently deteriorate after a series of treatment regimens. Physicians should observe the patientʼs clinical course precisely and decide on timely operative indications while collaborating with surgeons. Subtotal colectomy with ileostomy is recommended for surgical procedure including laparoscopic approaches in some cases. Operative morbidity is still high in spite of decreased mortality. This is because patients are placed on poor nutrition, anemia, and immunosupressed conditions.
