[ Summary ] GIST in the lower gastrointestinal tract exhibits SMT and sometimes develops in the rectum. GIST may be recognized with barium enama X-rays, colonoscopies, CTs, MRIs, or PET scans and so on. However, we can not often obtain a histological diagnosis of SMT. We can make a differential diagnosis between SMT and extracolonic compression. In addition EUS-FNA is very useful for the histological diagnosis of GIST. The primary treatment of GIST is complete surgical resection of the tumor. In cases with large tumors careful neoadjuvant therapy with imatinib should be considered as a component of strategies for GIST treatment in order to preserve anal function.
