[ Summary ] Hepatocellular carcinoma (HCC) has traditionally been considered a rare complication in patients with autoimmune hepatitis (AIH). The number of reported cases has however increased recently. The prevalence of HCC in patients with AIH was low in a recent nationwide survey. However, advanced fibrosis stage, particularly liver cirrhosis, is a major risk factor for the development of HCC. A focused surveillance strategy, based on imaging studies, is recommended for these patients. In addition, sustaining remission by implementing adequate immunosuppressive therapy may be important for prevention of the development of HCC in patients with AIH. Careful attention should be paid to the potential development of HCC in patients with AIH.
