[ Summary ] Transjugular intrahepatic portosystemic shunt (TIPS) has been performed to treat portal hypertension for approximately twenty years and has been one of the important therapeutic strategies for portal hypertension. Indications for use of TIPS are different for individual disease related to portal hypertension. Careful pre treatment investigation of systemic diseases, as well as liver and/or renal functions are needed to perform TIPS. Pre TIPS evaluation, employing imaging modalities increases the number of favorable technical outcomes. We can adjust stent diameters according to liver function, portal pressure and the clinical effects of TIPS. After TPS procedures, shunt dysfunction and hepatic encephalopathy may develop but all of the above respond to therapy. TIPS is effective for improving individual refractory diseases related to portal hypertension. Most patients who are suitable for TIPS are candidates for liver transplantation and must consider the necessity of transplantation.
