[ Summary ] Before the clinical application of NOTES can be established, substantial basic laboratory work under multi-disciplinary team driven by GI surgeons and physicians is necessary. Differences between laparoscopic surgery and flexible GI endoscopy must be clearly recognized during this process. This challenge, will lead us to a deeper understanding of current technical barriers related to NOTES. Problems encountered during clinical attempts at developing NOTES will be overcome by close collaborative efforts by surgeons and physicians.
