[ Summary ] Laparoscopic surgery has gained widespread acceptance because of its reduced abdominal wall trauma and because it has several advantages over conventional open surgery. With the acceptance of laparoscopic approachs, current efforts are aimed at reducing the size and number of trocar access points. Two recent innovations are natural orifice translumenal endoscopic surgery (NOTES) and single port surgery (SPS). In NOTES, a natural orifice serves as the entry point to intraabdominal organs. SPS is performed using the umbilicus as the single entry portal. Although clinical application of NOTES has already begun, human NOTES procedures require significant technological improvement. SPS can be performed using refinements to existing laparoscopic instruments. The number of cases employing SPS is rapidly increasing. To determine the future role of NOTES and SPS, further studies are required.
