[ Summary ] Due to recent progress in endoscopic submucosal dissection (ESD), most lesions can be resected in an en bloc fashion. The advantages of ESD are precise pathological diagnosis and low rates of local recurrence. In our institution, we first resect lesions which are determined to be deeper than T1a MM through employing ESD procedure. After precise diagnosis we decide on additional treatment for those at risk of lymph node metastasis. ESD is an excellent treatment in terms of function preservation and patientʼs QOL. However, superficial esophageal carcinoma deeper than T1a MM has a potential risk of lymph node metastasis, additional treatment should be decided on in carefully. Surveillance after treatment is also important. Lymph node metastasis generally occurs within two years of treatment. Therefore, more careful observations are necessary in this period.
