[ Summary ] The Narrow Band Imaging (NBI) system consists of a sequential electronic endoscopic system and a light source equipped with new narrow band filters, yielding very clear images of microvessels on mucosal surfaces. Magnifying endoscopy combined with narrow-band imaging (MENBI) has been used for differential diagnosis of various focal lesions. Magnifying endoscopy performed in combination with the NBI system is very usefull for diagnosing gastric neoplasia. Endoscopic resection (EMR and ESD) has been accepted worldwide as a less invasive standard treatment for early gastric cancer. Endoscopic submucosal dissection (ESD) facilitates en bloc resection with through histopathologic evaluation of specimens. It is very important to use ME-NBI system to perform en bloc resection, because this type of system enables us to improve our ability to differenciate between benign and malignant minute lesions. Additionally, when used in conjunction with conventional endoscopic diagnosis, one is able to more accurately diagnose the extension of cancerous infiltration.
