[ Summary ] Gastric perforation is a major complication associated with endoscopic submucosal dissection (ESD). Most perforations can be closed with Endoclips. However, in rare caces it is difficult to do so. In these cases we use Detachable Snares for Endoscopic suturing. Detachable Snares are tightened after they are fixed with Endoclips at two points. It is comparatively easy to close a peforation by using 2 or 3 Detachable Snares. Three cases employing endoscopic suturing with Detachable Snares were performed and all cases exhibited normal recovery rates. This is not a technique which should be used frequently, but is useful when perforations cannot be closed with Endoclips alone.
