Title Treatment ofGastroduodenal Bleeding Ulcers
Publish Date 2010/12
[ Summary ] Gastroduodenal ulcers are an important factor leading to gastrointestinal bleeding. Endoscopic treatment is useful for bleeding peptic ulcers. However, we must first manage the general condition of patients. An ulcer with active bleeding or having non-visible bleeding from vessels (Forrest classification Ia, Ib, IIa) is an indication for endoscopic treatment. Hemostatic treatments should be chosen for individual treatments depending on the situation. If the patient is at high-risk of rebleeding, we should take a second look within 24 hours since hemostatic treatments may be necessary. After endoscopic treatment, acid-reducing drugs should be administered for prevention of rebleeding. Eradication therapy is necessary for Helicobacter pylori (H.pylori) positive patients after ulcer healing has occured. Supporting treatments, such as blood transfusions, are important for management of the patientʼs general condition. Saving the lives of patients is the primary goal. Therefore, it is desirable to follow care guidelines while providing flexible care for individual patient needs.
