[ Summary ] Treatments for gastric or duodenal perforations start with decisions as to whether surgery is required. When peritonitis is more severe and the overall condition of the patient is declining, surgical procedures must be considered. When we choose a non-operative treatment, we must insert a gastric tube, administer antibiotics and PPIs, as well as strictly observe the patient's condition. A CT scan should be taken after 24 hours, and if the condition has not improved, operative treatment is recommended. The standard procedure is abdominal cavity drainage, along with closure of the perforated site, and an omental patch. Post-operative eradication of H. pylori is required.
