[ Summary ] Reflux of pancreatic juice into the bile duct (pancreatobiliary reflux) is observed in cases with a congenital anomaly, abnormal arrangement of the pancreatobiliary ductal system (AAPB), and is considered to be relevant to the development of biliary cancer. Recently, the number of reports on pancreatobiliary reflux with normal union of the pancreatic and bile ducts are increasing, and the correlation between reflux and the development of biliary malignancy in such patients is gaining attention. In case with AAPB, prophylactic surgery is recommended even though cancer development is clinically unclear. The necessity of prophylactic surgery for patients with pancreatobiliary reflux in a normal arrangement of the pancreatobiliary ductal system should be further examined.
