[ Summary ] The recent advances in rectal surgery are remarkable. Low anterior resection using stapled anastomosis rather than abdominoperineal resection is currently the primary procedure for rectal cancer. Intersphincteric resection for very low tumors, and new types of reconstruction such as colonic J-pouch are currently performed. Total mesorectal excision and preoperative radiotherapy or chemoradiotherapy have been shown to contribute to reductions in postoperative local recurrence. In Japan, lateral node dissection with preservation of pelvic autonomic nerves has been established as a primary treatment modality. The indications for minimally invasive surgery, such as local excision and laparoscopic surgery, have also been extended.
