Title Recent Medical Treatments for Esophageal Hemorrhage

[ Summary ] In Japan, technology for endoscopic diagnosis and medical treatment has been developed. In addition to individual treatments, a very high level of care is also provided through combined medical treatments to control esophageal bleeding. On the other hand, there has been insufficient examination concerning treatment of esophageal varices. The traditional beta blocker (propranolol) is covered by Japanese insurance for the purpose of preventing bleeding. However, recent overseas reports are attracting attention to angiotensin II receptor blockade (ARB). Suppression of the progression of portal vein pressure and of liver fibrosis has been reported. Evidence of reduced bleeding from esophageal varices, disease progress prevention, preventing liver cirrhosis and hepatic insufficiency as well as improving overall medical treatment is expected. For those receiving medical treatment while experience esophageal bleeding, the theraphy must be different than that for esophageal varices or other diseases. Treatment shouldbe divided roughly into the categories of urgent treatment and prophylactic treatment for patients with esophageal bleeding. We will describe these treatment options.
