[ Summary ] It is often necessary to perform ERCP (endoscopic retrograde cholangiopancreatography) on patients who have undergone surgical procedures that alter the upper gastrointestinal anatomy, especially in elderly patients. Endoscopists should fully understand this anatomy and select the appropriate scope and devices to successfully perform ERCP procedures. Endoscopic papillary balloon dilation (EPBD) is easy to perform in these patients without the risk of bleeding compared to endoscopic sphincterotomy (EST) . Balloon-enteroscope-assisted ERCP is a new and promising technique for patients with altered gastrointestinal anatomies. Delicate endoscopic maneuvers are important factors to achieve safe successful pancreaticobiliary procedures.
