[ Summary ] Dumping syndrome results from rapid delivery of food into the small bowel. It occurs primarily after gastrectomies and the symptoms are classified into early and late forms. Early dumping occurs in the first 30 minutes after meals and late dumping 1-3 hours postprandially. The clinical manifestations of early dumping include postprandial abdominal and systemic symptoms, resulting from osmotic fluid shifts and release of vasoactive humoral mediators. Late symptoms are secondary to reactive hypoglycemia induced by exaggerated release of insulin and GLP-1. Effective relief of symptoms can be achieved in the majority of patients with dietary and lifestyle modifications to minimize ingestion of simple carbohydrates. More severely affected individuals may need medications.
