[ Summary ] The indications for pancreatoscopy are diagnosis and evaluation of treatment of intraductal papillary mucinous tumors (IPMT), endoscopic treatment such as electronic hydrolic lithotripsy, pancreatic stones, diagnosis of pancreatic duct strictures, tumors, and other conditions. Before performing pancreatoscopies which with 2.6 - 3.4 mm incisions, we should consider whether the scope can reach to the target or not. One should also consider theraputic indications and bleeding tendencies, that is, whether a cathether scope (PF-8P) should be used when biopsy is not necessary. In case with IPMT, pancreatoscopy can be traverse without EST, however EST should be done in most cases, in order to reach the target. A “salmon egg appearance” may be observed in IPMT cases. When using NBI, it may become apparent one is observing vascular network in the pancreatic duct. We may then diagnose superficial IPMT spreading.
