[ Summary ] This article describes the guidelines for treatment of Crohn's disease made in 2007 by the Research Committee of Inflammatory Bowel Disease, of the Japanese Ministry of Labor and Welfare. It comments on the revised positions, compared to those made in 2002. The main changes were as follows ; (1) Infliximab, a thimeric antibody to tumor necrosis factor (TNF) - α, was adopted as a medicinal therapy, (2) ciprofloxacin was added as an antibiotic therapy, and (3) administration of Infliximab for active anal lesions with a lack of or good control of perianal abscess, was described. In this guideline, infliximab is indicated for patients who do not respond or inadequately respond to nutritional and / or other medicinal therapies. Based on a nationwide multicenter survey conducted by questionnaire as a part of a project the Japanese Ministry of Health and Welfare, several characteristics concerning the therapeutic efficacy of Infliximab were also described in the guidelines.
