[ Summary ] Previous reports concerning long-term outcomes for surgical intervention to control intractable pain due to chronic pancreatitis are summarized. The ratio of pain relief provided by conventional longitudinal pancreatico-jejunostomies was around 80 %, that achieved through Frey or Beger operations was reported to be about 90 %. This indicates that interventions for pancreatic head treatment are effective in some patients. Recent results of RCTs comparing endoscopic pancreatic stenting with surgical pancreatico-jejunostomies clearly showed the superiority of surgical operations in providing long-term effects. In patients with dilated ducts, surgical drainage operations should be selected as the standard therapy for treatment of chronic pancreatitis.
