[ Summary ] Endoscopic submucosal dissection (ESD) is an effective treatment for early gastrointestinal cancer, particularly for early gastric cancer. Endoscopic mucosal resection (EMR) is a procedure that has been used widely since 1990. It is a safe and effective procedure. EMR has the advantage of short operative times. If the conditions could be established which would make EMR curative resection rates the same as those of ESD, then it would be possible to apply each procedure to particular cases. The outcome of EMR was not as good as that of ESD when we examine lesions with indications for the procedure per the Gastric Cancer Treatment Guidelines or spreading lesions with indications for the procedure per guidelines at our hospital. However, when our past cases were examined in detail by lesion sites and sizes, the outcome of EMR was comparable to that of ESD if the tumor size was 5 mm or less at any site, or 10 mm or less in the L area. By identifying the conditions under which reliable one-piece resection can be performed, it is considered possible to apply each of these procedures for different uses.
